Description

Mechanism of Action
Cagrilintide is an acylated, long-acting analog of the pancreatic hormone amylin that signals satiety through amylin and calcitonin receptors, slowing gastric emptying and reducing food intake. Semaglutide is a GLP-1 receptor agonist that enhances glucose-dependent insulin secretion, suppresses glucagon, and reduces appetite via central GLP-1 receptors. Clinical Research: The REIMAGINE Phase 3 Program
In the phase 3a REIMAGINE 1 trial, adults with early type 2 diabetes inadequately controlled on diet and exercise received once-weekly cagrilintide-semaglutide for 40 weeks. The 2.4 mg + 2.4 mg dose lowered HbA1c by 1.8 percentage points and reduced body weight by 13.8%, versus 0.1 points and 1.4% with placebo (Aroda et al., Lancet Diabetes Endocrinol 2026).
Across the wider REIMAGINE program, CagriSema outperformed semaglutide alone: in REIMAGINE 2 it reduced HbA1c by 1.91 points versus 1.76 with semaglutide and produced 14.2% weight loss versus 10.2%, while in REIMAGINE 3 — added to basal insulin — it lowered HbA1c from 8.8% to 6.5% with weight reductions up to 12% and no severe hypoglycemia (HCPLive, 2026).
